Overview

These are the two most common modifiers used in PM&R to bypass bundling edits. Misuse is a top audit trigger.

Modifier 25

  • Definition: Significant, Separately Identifiable E/M Service by the Same Physician on the Same Day.
  • Usage: Attached to an E/M Code (99202-99215).
  • Scenario: Patient comes for follow-up (E/M) AND receives a Trigger Point Injection (Procedure).
  • Requirement: The E/M must be above and beyond the usual pre/post work of the procedure.

Modifier 59 (or XE/XS/XU)

  • Definition: Distinct Procedural Service.
  • Usage: Attached to a Procedure Code (e.g., 97140, 20552).
  • Scenario: Two procedures performed that are usually bundled, but done on different sites or separate sessions.
  • Example: Manual therapy (97140) on the knee AND manual therapy (97140) on the shoulder (different anatomical sites).

Medicare Preference

Medicare prefers X-Modifiers (XE, XS, XP, XU) over 59 for greater specificity. Use XS for separate anatomical sites.
