H47.331: Pseudopapilledema of Optic Disc, Right Eye
Clinical Context
Pseudopapilledema refers to an elevation of the optic disc that mimics true papilledema (swelling due to increased intracranial pressure) but is actually caused by benign anatomical anomalies. The most common cause is Optic Disc Drusen (ODD), which are calcified deposits buried within the optic nerve head.
Unlike true papilledema, pseudopapilledema is not associated with raised intracranial pressure. Accurate coding and diagnosis are critical to avoid unnecessary, invasive testing (e.g., lumbar puncture) for the patient.
ICD-10-CM Diagnosis Codes
Primary Code
H47.331 - Pseudopapilledema of optic disc, right eye
Usage: Use this code when the clinician confirms the swelling is “pseudo” (false) and restricted to the right eye.
Related & Differential Codes
H47.332 - Pseudopapilledema of optic disc, left eye
H47.333 - Pseudopapilledema of optic disc, bilateral
H47.321 - Drusen of optic disc, right eye
Usage: Use if the specific cause (drusen) is identified and documented.
H47.10 - Papilledema, unspecified eye
Usage: Use only if the diagnosis is truly elevated intracranial pressure or if the distinction between “pseudo” and “true” is not yet determined.
CPT Procedure Codes (Workup & Monitoring)
1 . Optical Coherence Tomography (OCT)
Code: 92133
Long Title: Scanning computerized ophthalmic diagnostic imaging, posterior segment, with interpretation and report, unilateral or bilateral; optic nerve.
- Explanation: This is the gold standard for differentiation. It measures the Retinal Nerve Fiber Layer (RNFL) thickness and can visualize “lumpy-bumpy” buried drusen vs. the smooth edema of true papilledema.
- wRVU (approx): 0.43
- Global Period: XXX (Diagnostic test; no global period)
- Assistant Payable: No
- NCCI Edit: See Bundling section below.
2 . Fundus Photography
Code: 92250
Long Title: Fundus photography with interpretation and report.
- Explanation: Used to establish a visual baseline. Drusen often become more visible on the surface over decades; photos track this progression.
- wRVU (approx): 0.40
- Global Period: XXX (Diagnostic test; no global period)
- Assistant Payable: No
3 . Visual Field Examination
Code: 92083
Long Title: Visual field examination, unilateral or bilateral, with interpretation and report; extended examination (e.g., Goldmann visual fields with at least 3 isopters plotted and static determination within the central 30°).
- Explanation: Optic disc drusen can cause peripheral vision loss (commonly nasal steps or arcuate defects) due to compression of nerve fibers. Extended testing is required to map these defects.
- wRVU (approx): 0.50
- Global Period: XXX (Diagnostic test; no global period)
- Assistant Payable: No
Bundling & NCCI Edits
Mutually Exclusive Pairs
- 92133 (OCT) and 92250 (Fundus Photos):
- These are frequently bundled. NCCI edits often carry a “Mutually Exclusive” flag.
- Modifier 59/X{EPSU}: Some payers allow unbundling if the procedures are performed for distinct reasons (e.g., OCT for the nerve, Photos for a separate retinal pathology), but for the same diagnosis (Pseudopapilledema), billing both on the same day is often denied or flagged for audit.
- Best Practice: If both are medically necessary, document clearly why one test alone was insufficient.
Inclusives
Bilateral Indicator: Codes 92133, 92250, and 92083 are defined as “unilateral or bilateral.”
Warning
Do not bill with modifier -50 or use RT/LT lines for payment adjustment; the fee is the same whether one or both eyes are tested.
Risk Adjustment (HCC)
- HCC Category: None.
- Note: H47.331 is generally considered a benign structural anomaly and does not risk-adjust for CMS-HCC models.
