Meaning

Modifier -55 is defined as Postoperative Management Only. It is used when one physician performed the surgery, but you (or the physician you are coding for) are providing the follow-up care during the global period.

Common Use Cases

  • The Returning Patient: A patient has a complex procedure at a teaching hospital (like the Medical College of Wisconsin) but returns to their local specialist for the 10-day or 90-day post-op follow-up.

  • Transfer of Care: A surgeon becomes unavailable (illness, vacation) and formally transfers the postoperative management to a colleague or partner.

Key Rules to Remember

  • Dates Matter: When billing -55, you must include the Date of Surgery in the electronic claim (usually Item 15 or 19 on a CMS-1500) and the date you assumed care.

  • The “Days” Calculation: Reimbursement is often pro-rated based on how many days of the global period you actually covered. If the global period is 90 days and you took over on day 30, you only bill for the remaining 60 days.

  • Global Period: This modifier is only necessary for procedures with a 10-day or 90-day global period. Minor procedures with a “000” global period don’t require it.
