Modifier XS - Separate Structure

A service that is distinct because it was performed on a separate organ or anatomical structure.
Long Definition
Modifier XS is a HCPCS Level II modifier and part of the X{EPSU} modifier family — CMS-created subsets of modifier 59. Modifier XS is used when two procedures that are normally bundled by NCCI edits were performed on different, distinct anatomical structures or organs during the same encounter, by the same provider, on the same date of service.
The rationale is that when the same CPT code or a related code is performed bilaterally or on separate anatomical sites/structures, the work involved is genuinely distinct — not duplicative — and therefore should not be bundled as though a single structure was treated.
When to Use
- The same or related procedure is performed on two different anatomical structures during the same encounter.
- The structures are clinically and anatomically distinct (e.g., two separate lesions, bilateral structures, different organ systems).
- NCCI edits would otherwise bundle the codes despite the procedures being on separate structures.
- Per CMS, modifier 59 or XS is appropriate for different anatomic sites during the same encounter only when the CPT code specifically identifies the procedure as one that can be performed on separate structures and the claim would otherwise be bundled.
When NOT to Use
- When the procedures were performed on the same anatomical structure — the NCCI edit stands and the services should remain bundled.
- When the distinctness is about timing (use XE) or provider identity (use XP) rather than anatomy.
- When a bilateral modifier (-50) is more appropriate than XS.
- Do not use XS and modifier 59 on the same claim line.
- Do not append to E/M services.
- Do not use XS simply because two incisions were made in the same general body region — structures must be genuinely separate organs or anatomical sites.
Documentation Requirements
- The operative or procedure note must clearly identify each separate structure treated and document distinct procedures at each site.
- Anatomical specificity matters — document exact location, laterality, or organ for each service billed.
- Medical necessity for treating each separate structure must be supported.
- Avoid vague documentation such as “multiple sites” without specifying the exact structures.
