Phase 1: Foundation Building (Weeks 1-4)

Focus: Transition from outpatient to inpatient mindset

Week 1-2: ICD-10-CM Deep Dive for Inpatient

  • Study Focus:
    • Principal diagnosis selection rules
    • Secondary diagnosis sequencing
    • Present on Admission (POA) indicators
    • UHDDS (Uniform Hospital Discharge Data Set) guidelines

    • Practice 20 scenarios: identifying principal vs. secondary diagnoses

Week 3-4: ICD-10-PCS Introduction

  • Study Focus:
    • PCS structure (7 characters)
    • Medical and Surgical section (the big one!)
    • Root operations and their definitions
    • Body parts and approaches
  • Study Tasks (3 hrs/week):
    • Memorize root operation definitions
    • Learn PCS tables navigation
    • Understand approach types (Open, Percutaneous, etc.)
  • Practice (2 hrs/week):
    • Build 30+ PCS codes from operative reports
    • Focus on common procedures: appendectomy, cholecystectomy, hip replacement

Phase 2: Core Competency (Weeks 5-10)

Focus: Master the most tested content areas

Week 5-6: MS-DRGs (Medicare Severity DRGs)

  • Study Focus:
    • DRG grouping logic
    • CC (Complication/Comorbidity) and MCC (Major CC) impact
    • MDC (Major Diagnostic Categories)
    • DRG optimization principles
  • Study Tasks (2.5 hrs/week):
    • Learn top 20 DRGs by volume
    • Understand CC/MCC exclusion lists
  • Practice (2.5 hrs/week):
    • Work through DRG assignment exercises
    • Compare cases with/without CCs

Week 7-8: Advanced ICD-10-PCS

  • Study Focus:
    • Obstetrics section
    • Other common sections: Administration, Measurement & Monitoring
    • Multiple procedure coding rules
    • Device coding
  • Study Tasks (2 hrs/week):
    • Review less common root operations
    • Study device value tables
  • Practice (3 hrs/week):
    • Code 25 complex operative reports
    • Practice cesarean sections, cardiac cath procedures

Week 9-10: Inpatient Documentation & Compliance

  • Study Focus:
    • Query practices (when to query physicians)
    • Medical necessity for inpatient admission
    • 72-hour rule and readmissions
    • PEPPER reports and audit risks
  • Study Tasks (2 hrs/week):
    • Study CMS guidelines on inpatient-only procedures
    • Learn compliant query formats
  • Practice (3 hrs/week):
    • Identify documentation deficiencies in sample charts
    • Write practice queries

Phase 3: Specialty Areas (Weeks 11-13)

Focus: High-value specialty coding

Week 11: Surgical Coding Deep Dive

  • Orthopedic procedures (joint replacements, spinal fusions)
  • Cardiovascular procedures (CABG, valve replacements, stents)
  • GI procedures (bariatric surgery, colectomies)
  • Practice: 20 complex surgical cases

Week 12: Medical Coding Mastery

  • Sepsis and severe sepsis coding
  • Respiratory failure and mechanical ventilation
  • Diabetes with complications
  • Heart failure and MI coding
  • Practice: 20 medical cases

Week 13: OB, Newborn, and Pediatrics

  • Delivery coding (vaginal vs. cesarean)
  • Obstetric complications
  • Newborn coding rules
  • Pediatric-specific conditions
  • Practice: 15 OB/newborn cases

Phase 4: Intensive Practice (Weeks 14-16)

Focus: Speed, accuracy, and exam simulation

Week 14-15: Full Case Coding

  • Practice (5 hrs/week):
    • Code 40-50 complete inpatient charts
    • Mix of medical and surgical cases
    • Time yourself: aim for 15-20 minutes per case
    • Review all answers thoroughly

Week 16: Mock Exams

  • Practice (5 hrs/week):
    • Take 2 full-length practice exams (2.5 hrs each)
    • Review incorrect answers
    • Identify weak areas and review those sections

Week 17: Final Review (Week before exam)

Focus: Confidence building and light review
