CMS Criteria
Patient must have:
- Vital organ system failure
- High risk of mortality
- Requires constant physician attention

- Communication with family (about care)
- Care coordination
NOT Included
- Procedures with separate CPT codes
- Teaching activities
- Travel time
- Administrative tasks
