DEFINITION of ostomy

An ostomy is a surgically created opening that allows bodily waste to exit through the abdominal wall into an external collection pouch when normal elimination pathways are compromised. The procedure involves bringing a portion of an internal organ (typically intestine or urinary tract) through the abdominal wall to create a stoma.

Types of Ostomies:

  • Colostomy: Opening from the colon (large intestine); stool consistency varies by location (ascending, transverse, descending, sigmoid).
  • Ileostomy: Opening from the ileum (small intestine); produces liquid to paste-like output.
  • urostomy (Ileal Conduit): Opening from the urinary tract; diverts urine when bladder is removed or non-functional.
  • Continent Ostomy: Internal reservoir with valve mechanism allowing intermittent catheterization (e.g., Kock pouch, Indiana pouch).
  • Temporary vs. Permanent: Temporary ostomies allow healing of distal bowel and may be reversed; permanent ostomies are lifelong.
  • Indications: Colorectal cancer, inflammatory bowel disease (Crohn’s, ulcerative colitis), diverticulitis, bowel obstruction, trauma, congenital anomalies (imperforate anus), bladder cancer, neurogenic bladder.
  • Management: Requires specialized ostomy appliances (pouching systems), skin barrier protection, and often consultation with a WOC (Wound, Ostomy, Continence) nurse.

ETYMOLOGY of ostomy

greek Derived from Greek roots:

  • Ostom-: From Greek stoma, meaning “mouth” or “opening.”
  • -y: Suffix indicating a procedure or condition.
  • Literally translates to “creating an opening” or “mouth-like opening.”
  • Related to -stomy (the procedure suffix), -stoma (the actual opening), and stomat- (mouth-related terms).
  • The term has been used in medical literature since the late 19th century as surgical techniques for bowel diversion developed.

Related Terms

  • Colostomy: Ostomy from the colon.
  • Ileostomy: Ostomy from the ileum (small intestine).
  • urostomy: Ostomy for urinary diversion.
  • Jejunostomy: Ostomy from the jejunum (feeding tube access).
  • Gastrostomy: Opening into the stomach (feeding tube).
  • Tracheostomy: Opening into the trachea (airway access).
  • Stoma: The visible opening on the abdominal surface.
  • Ostomy Appliance/Pouch: External collection device worn over the stoma.
  • Wafer/Barrier: Skin-protective adhesive base for the ostomy pouch.
  • Ostomy Reversal: Surgical procedure to close the ostomy and restore normal continuity.
  • Peristomal Skin: Skin surrounding the stoma; prone to irritation and complications.
  • WOC Nurse: Wound, Ostomy, Continence nurse specialist.
  • ** Continent Catheterizable Stoma:** Internal reservoir allowing self-catheterization.
  • Loop Ostomy: Temporary ostomy with a loop of bowel brought to surface.
  • End Ostomy: Permanent ostomy with one end of bowel brought to surface.

Important Coding Notes:

  • Status vs. Complication: Use Z93 codes for presence of ostomy without complications; use K94 codes for ostomy complications.
  • Underlying Condition: Always code the underlying condition requiring the ostomy (cancer, IBD, obstruction, etc.) in addition to the ostomy status code.
  • Temporary vs. Permanent: Documentation should specify if ostomy is temporary or permanent; affects prognosis and care planning.
  • Ostomy Care: WOC nurse consultations and ostomy education may be billed under E/M codes or specific care management codes.
  • Supplies: Ostomy supplies are typically covered under Medicare Part B or DME (Durable Medical Equipment) benefits with HCPCS codes.
  • Reversal: Ostomy closure/reversal procedures require documentation of healing and readiness for restoration of continuity.
  • Bilateral: Ostomies are typically midline/unilateral; bilateral codes rarely apply.
  • Inpatient vs. Outpatient: Most ostomy creations are inpatient procedures requiring ICD-10-PCS codes for facility billing.
