Modifier 57, Decision for Surgery, is a powerful tool in your coding arsenal, especially for the high-stakes surgeries you’ll encounter in Urology, Ophthalmology, and Otolaryngology at the Medical College of Wisconsin.
Unlike Modifiers -54 and -55, which split the “work” of a surgery, Modifier 57 identifies the Evaluation and Management (E/M) service where the surgeon first determined that a major operation was necessary.

The “90-Day Rule” (Major Surgery Only)
The most critical thing to remember is that Modifier 57 is exclusively for major surgeries (those with a 90-day global period).
- If you use it on a minor procedure (0 or 10-day global), the claim will likely be denied. For minor procedures, you should look at Modifier -25 instead.
Timing is Everything
To use Modifier 57, the E/M visit must occur on:

Documentation Requirements
Auditors look for the “Initial Decision.” Your documentation must clearly state that the surgery was not planned prior to this specific encounter. If the patient was already scheduled for surgery and this was just a “pre-op clearance” visit, Modifier 57 is not appropriate.
