Always confirm lid position in the operative note before selecting the E modifier.

  • Using -LT instead of -E2: Modifier -LT alone does not distinguish the lower from the upper eyelid. -E2 must be used for lower left eyelid-specific procedures โ€” -LT is insufficient for eyelid claims.
  • Combining multiple lid procedures on one line: Each lid must be a separate billing line. A single line with -E2 only covers the lower left lid; if other lids are also treated, additional lines with -E1, -E3, or -E4 are required.
