Cataract Coding Note β€” Most Critical Distinctions:

  • 66984 = standard cataract; 66982 = complex cataract (must document complexity β€” IFIS, pseudoexfoliation, subluxation, small pupil, dense brunescent, prior vitrectomy); 66991 = cataract + MIGS same session.
  • Premium IOL (toric, EDOF, multifocal) β€” report 66988/66989 for the procedure; patient pays the IOL upgrade as a non-covered service; do not report the IOL as a separate supply.
  • YAG capsulotomy (66821) has a 90-day global included in the cataract surgery global period β€” do NOT bill 66821 within 90 days of the cataract extraction.
