  • Each sinus code is reported separately for the sinuses treated.
  • Modifier -50 when the same sinus procedure is performed bilaterally.
  • Modifier -51 for the secondary lower-value procedure when multiple different FESS codes are reported.
  • 31231 (diagnostic endoscopy) is NOT separately billable when any surgical endoscopy code is used β€” bundled.
  • Balloon sinuplasty (31295-31298) and conventional FESS codes (31254-31288) may be reported together when both approaches are used in the same session for different sinuses.
